Prenatal Case Management
This program is focused toward reducing infant mortality
and low birth weight by reducing inadequate prenatal care. Clients eligible for
this program are contacted at least monthly by a registered nurse to ensure they
are keeping appointments with their health care provider and following recommendations
and instructions.
